<p>Those startling results, published online Monday in the journal Pediatrics, are only an estimate. But several experts who reviewed the analysis said the methods and conclusions seem sound.</p>

<p>With the passage of Obamacare, creating a vast new middle-class entitlement, a national sales tax of the kind near-universal in Europe is inevitable.</p>
<p><a href="http://www.treasurydirect.gov/govt/reports/pd/mspd/2010/opds022010.pdf">We are now $8 trillion in debt</a>. The Congressional Budget Office projects that $12 trillion will be added over the next decade. Obamacare, when stripped of its budgetary gimmicks &#8212; the unfunded $200 billion-plus &#8220;doctor fix,&#8221; the double counting of Medicare cuts, the 10-6 sleight-of-hand (counting 10 years of revenue and only six years of outflows) &#8212; is at minimum a $2 trillion new entitlement.</p>
<p>It will vastly increase the debt. But even if it were revenue-neutral, Obamacare preempts and appropriates for itself the best and easiest means of reducing the <em>existing</em> deficit. Obamacare&#8217;s $500 billion of cuts in Medicare and $600 billion in tax hikes are no longer available for deficit reduction. They are siphoned off for the new entitlement of insuring the uninsured.</p>

<p>There are no cuts to the traditional Medicare benefit. The lion&#8217;s share of spending cuts are in Medicare Advantage &#8212; a program that uses private firms such as Humana and UnitedHealth Group to deliver Medicare benefits. Many of these providers offer extra coverage and some of those extras could be dropped as Medicare Advantage subsidies are bought more in line with the cost of traditional Medicare benefits. Medicare Advantage payment rates will be frozen in 2011 and then gradually reduced giving companies time to adjust to the changes.</p>

<p>Many big provisions don&#8217;t kick in until 2014, including the mandate for most folks to have health insurance and many new requirements for health-plan designs. Before then, you&#8217;ll see a mishmash of other things go into effect at various times—and of course some of the changes depend on the Senate passing the House&#8217;s so-called sidecar, or reconciliation, bill of changes.</p>
<p>Here are some ways you can start dealing with the new health-care landscape.</p>
<p><strong>Do your homework.</strong> This legislation will almost certainly affect your wallet and your health coverage, so you need to understand it. The Kaiser Family Foundation&#8217;s site, kff.org, has a <a href="http://kff.org/healthreform/sidebyside.cfm" target="_blank">side-by-side bill comparison tool</a> featured on the main page, and you can choose the Senate and reconciliation bills, selecting only the parts you care about.</p>
<p>Congressional Democrats offer their own explainers, including a timeline, at energycommerce.house.gov. Click on the &#8220;Affordable health care for America&#8221; button on the right.</p>
